Recognizing that we can’t be all things to all people, Common Ground Psychiatry does not treat:
Individuals younger than age 18 or older than age 64
Individuals with active substance use disorders
Individuals without insurance (we need to ensure you have access to medications and lab monitoring)
Individuals on Medicaid (Colorado laws require you to receive in-network care)

Insurance
Common Ground Psychiatry does not contract with insurance companies, meaning Dr. Czaban is an out-of-network provider. Payment is collected at the time of service. You will receive a billing statement in the portal to submit to your insurance company for possible reimbursement. However, we cannot guarantee any reimbursement.

Psychiatric evaluation (90 minutes): $490
Follow-up appointment (up to 30 minutes): $180
Follow-up appointment (30-40 minutes): $240
Follow-up appointment (40-50 minutes): $300
Follow-up appointment (50-60 minutes): $360
Extended phone support (15 minutes): $95
