Our online portal makes your care seamless! You can:

  • Directly email Dr. Czaban

  • Schedule and change follow-up appointments

  • Complete and sign forms electronically

  • Make payments and obtain billing statements

  • Receive email reminders of appointments

The Psychiatric Evaluation

The purpose of the Psychiatric Evaluation is to clarify your diagnosis, assess your treatment needs, and ensure Common Ground Psychiatry and you are a good fit. If you proceed with treatment, Dr. Czaban will design a treatment plan with you and, as appropriate, begin prescribing medication. If you are currently taking psychiatric medication, please ask your prescriber to provide refills until Dr. Czaban can begin treatment.

Insurance

Common Ground Psychiatry does not contract with insurance companies, meaning Dr. Czaban is an out-of-network provider. Payment is collected at the time of service. You can submit a billing statement to your insurance company for possible reimbursement, but we cannot guarantee any reimbursement.

Rates

  • Psychiatric evaluation (90 minutes): $490

  • Follow-up appointment (up to 30 minutes): $180

  • Follow-up appointment (30-40 minutes): $240

  • Follow-up appointment (40-50 minutes): $300

  • Follow-up appointment (50-60 minutes): $360

  • Extended phone support (15 minutes): $95
